Healthcare systems, in alignment with Saudi Vision 2030, are undergoing a shift toward more integrated, accessible, and patient-centered models of care that extend beyond hospital settings into homes and communities. This transformation places strong emphasis on early intervention, chronic disease management, continuity of care, and improved health outcomes through home-based and community-oriented services.

Healthcare professionals increasingly face the need to deliver safe, effective, and coordinated care in non-traditional settings while ensuring patient engagement and family involvement in care planning. However, challenges remain in applying patient-centered care principles consistently, coordinating across care settings, and addressing the diverse health needs of populations within the community.

There is also a growing need to strengthen competencies in home care delivery, communication with patients and families, interprofessional collaboration, and population health approaches. Enhancing these skills is essential to improve accessibility, reduce hospital admissions, and promote equitable health outcomes.

This workshop addresses these needs by equipping participants with practical knowledge, evidence-based approaches, and applied strategies to deliver high-quality home-based and patient-centered care aligned with national health priorities.
